REGISTRATION FORMS (FOUND TO THE RIGHT)

Families can click on the link below download the fillable registration forms or the view the document on the right. Once completed you can email the forms to principal@stpiusxcatholic.org. All new families please deliver the following things to the office upon registration:

• Copy of Legal Birth Certificate
• Copy of Baptismal Certificate (if baptized)
• Social Security Number (Lansing Diocese MUST have)
• Proof of Up-to-date Immunizations
• Students entering Kindergarten – Michigan Law (House
• Bill 5094) requires parents of students entering kindergarten to present a statement confirming that they have received the Michigan Department of Community Health Vision Screening or an eye examination by a doctor prior to entering school.
